Referral process

In order to attend Oakleaf, you must be:

  • Aged 16-65
  • Interested in developing work-related skills
  • Motivated to attend regularly

You can refer yourself to Oakleaf by downloading and completing this form, or you can ask one of the following to refer you using this form:

  • Your GP
  • Your psychiatrist
  • Your key worker e.g. Social Worker, CPN or STR Worker)
  • A member of staff from a voluntary or statutory agency

Once we have received your referral form, we will invite you to come in for an initial discussion where we will jointly develop a preliminary plan of how we can best support you and develop your skills, alongside which we will help you monitor your progression. This is nothing to be worried about; our staff have lots of experience of working with mental health sufferers and understand that you may be nervous. Part of this process will be assigning you a client services adviser who will support you through your journey at Oakleaf.

If you find that you are unhappy with your choice of training course, you will be able to change to a different department, dependent upon availability. Your progress will be monitored regularly through reviews so you will be able to see how well you are doing.

We aim to arrange an appointment with you within 2 working days of receiving your referral and to follow this up with your first appointment within 14 days.
