Referral Process

In order to attend Oakleaf, you should be:

  • Aged 16-65
  • Interested in developing work-related skills
  • Or, interested in:

    • Making friends
    • Participating in social and leisure activities
    • Staying well

You can refer yourself to Oakleaf by downloading and completing our Referral Form, or you can ask one of the following to refer you using this form:

  • Your GP
  • Your psychiatrist
  • Your key worker e.g. Social Worker, CPN or Enabling Independence Team Worker)
  • A member of staff from a voluntary or statutory agency

Please send via email your completed form to or send it to our Client Services Department: Oakleaf Enterprise, 101 Walnut Tree Close, Guildford, Surrey GU1 4UQ. 

Once we have received your referral form, we will invite you to come in for an initial discussion where we will talk through how we can best support you and develop your skills, alongside which we will help you monitor your progression. This is nothing to be worried about; our staff have lots of experience of working with people struggling with their mental health and understand that you may be nervous. Part of this process will be assigning you a client services adviser who will support you through your journey at Oakleaf.

Your progress will be monitored regularly through reviews so you will be able to see how well you have stabilised or are progressing .

We aim to arrange an appointment with you within 5 working days of receiving your referral form and to follow this up with your first meeting with us within 14 days.
