How do I become a client?

To become a client you must go through a referral process.

You can either refer yourself  by completing the online form below or you can ask one of the following services to refer you, using the same form, below

A GP | A Psychiatrist | A Key Worker (e.g. Social Worker, CPN or Enabling Independence Team Worker)

Once we have received your referral form, we will invite you to come in for an initial discussion where we will talk through how we can best support you and develop your skills.


This is nothing to be worried about; our staff have lots of experience of working with people struggling with their mental health and understand that you may be nervous. Part of this process will be assigning you a Client Services Adviser who will support you through your journey at Oakleaf.


We conduct regular reviews so you will be able to see how well you are progressing with your recovery.


We aim to arrange an appointment with you within 5 working days of receiving your referral form and to follow this up with your first meeting with us within 14 days.
