Bridge the Gap

‘Bridge the Gap’ is a trauma-informed outreach service supporting individuals facing multiple disadvantages in Surrey. As part of the Government’s Changing Futures initiative, we provide person-centred key worker support, developed with lived experience experts. For more information, click here.

We provide 1:1 support and flexible hours: Mon–Fri (10am–8pm), Sat–Sun (9am–12pm), helping those affected by: mental health, homelessness, substance use, domestic abuse, or contact with the criminal justice system. We assist with housing, healthcare, finances, harm reduction, and referrals, empowering individuals to rebuild their lives.

Bridge the Gap clients can also access the Surrey Adults Matter (SAM) approach – a multi-agency ‘Team Around the Person’ (TAP) process. This brings together professionals to support individuals, identify challenges, and create a personalised action plan. Practitioners can refer individuals via our referral form or directly to SAM.

Check Point Case Worker

Check Point Programme

Working in partnership with Surrey Police our Check Point programme is focused on supporting individuals involved in the criminal justice system through a trauma informed approach. Work is completed in a time limited framework with an aim of reducing re-offending through restorative justice. 

Assertive Outreach Programme

From September 2026, Oakleaf will deliver a new Surrey-wide outreach service in partnership with Catalyst Support and The Hope Hub. The service provides up to 24 weeks of one-to-one, trauma-informed support for adults experiencing multiple disadvantage, including mental ill-health alongside challenges such as housing insecurity, substance misuse, unemployment or involvement with the criminal justice system.
