I Need Help
To become a client you must be an adult (16+) and interested in one or more of the following: Developing work-related skills, reducing isolation, participating in activities to gain confidence and skills, looking after your mental health and wellbeing.
You also must go through a referral process.
You can either refer yourself by completing the online form below or you can ask one of the following services: GP, Psychiatrist, Key Worker (Social Worker, CPN or Enabling Independence Team Worker) to refer you using the same form below.
Once we have received your referral form, we will invite you to come in for a welcome meeting where we will talk through how we can best support you and develop your skills.
This is nothing to be worried about; our staff have lots of experience working with people who are struggling with their mental health and understand that you may be nervous. Part of this process will be assigning you a Client Services Advisor who will support you through your journey at Oakleaf.
We conduct regular reviews so you will be able to see how well you are progressing with your recovery.

We offer a wide range of services to empower you to gain the skills and confidence needed to manage your mental health and achieve your goals:
Designed to build confidence, physical health and wellbeing; reducing isolation and loneliness.
Courses in IT and Gardening can help you to gain the skills and qualifications needed to return to the world of work.
Our ‘Path to Success’ Employment Project delivers tailored employment training and mentoring for clients.
You can access counselling sessions with our BACP-registered service to aid your recovery.

CHANGING FUTURES: BRIDGE THE GAP
Bridge the Gap is a specialist ‘trauma informed’ outreach service (part of the ‘Changing Futures’ Government Initiative) that has been developed in conjunction with lived experience experts and alliance members to offer person-centred key worker support to individuals experiencing multiple disadvantage in Surrey.
Oakleaf is just one delivery partner of the Bridge the Gap Alliance. We provide support on a one-to-one basis, 4-8 hours per week per client, 10am-8pm Monday to Friday, 9am-12pm on weekends.
Our team covers areas including but not limited to: Contact with the criminal justice system, mental health issues, substance use (alcohol or drugs), homelessness or at risk of becoming homeless, domestic violence/abuse.

SAFE HAVEN
The Guildford Safe Haven is based at our Oakleaf premises and provides out-of-hours support for people and their carers who are experiencing a mental health crisis or emotional distress.
Open 6pm-11pm, 365 days a year
