I Need Help

How Do I Become A Client

person icon

To become a client you must be an adult (16+) and interested in one or more of the following: Developing work-related skills, reducing isolation, participating in activities to gain confidence and skills, looking after your mental health and wellbeing.

You also must go through a referral process.

You can either refer yourself by completing the online form below or you can ask one of the following services: GP, Psychiatrist, Key Worker (Social Worker, CPN or Enabling Independence Team Worker) to refer you using the same form below.

Once we have received your referral form, we will invite you to come in for an initial discussion where we will talk through how we can best support you and develop your skills.

This is nothing to be worried about; our staff have lots of experience working with people who are struggling with their mental health and understand that you may be nervous. Part of this process will be assigning you a Client Services Adviser who will support you through your journey at Oakleaf.

We conduct regular reviews so you will be able to see how well you are progressing with your recovery.

If you wish to discuss the referral process with a member of Oakleaf staff, please either call us on 01483 303649 or alternately email: info@oakleaf-enterprise.org.
