How Do I Become A Client

To become a client, you must be 16+ and interested in developing work-related skills, reducing isolation, building confidence, or improving mental well-being. A referral is required – you can either self-refer via the form below or ask a GP, psychiatrist, or key worker (Social Worker, CPN, or Enabling Independence Team Worker) to do so.

After receiving your referral, we’ll invite you to a welcome meeting to discuss how we can support you. Our experienced staff understand that you may feel nervous. You’ll be assigned a Client Services Advisor to guide you through your journey at Oakleaf, with regular progress reviews.

BRIDGE THE GAP

‘Bridge the Gap’ is a trauma-informed outreach service supporting individuals facing multiple disadvantages in Surrey. As part of the Government’s ‘Changing Futures’ initiative, we provide person-centred key worker support, developed with lived experience experts.

We provide 1:1 support (4–8 hours per week) and flexible hours: Mon–Fri (10am–8pm), Sat–Sun (9am–12pm), helping those affected by: mental health challenges, homelessness, substance use, domestic abuse, or contact with the criminal justice system. We assist with housing, healthcare, finances, daily living, harm reduction, and referrals, empowering individuals to rebuild their lives. For more information, click here.

SAFE HAVEN

Located at Oakleaf, the Guildford Safe Haven offers out-of-hours support for anyone experiencing a mental health crisis or emotional distress.

Opening Hours: 6pm–11pm, 365 days a year
Location: 101 Walnut Tree Close, Guildford, GU1 4UQ
Drop-in only (no phone line available) – no appointment needed

The Guildford Safe Haven is run in partnership with Catalyst SupportSurrey and Borders Partnership (SABP) NHS Trust, and Oakleaf and is one of five Safe Haven locations in Surrey and North East Hampshire.
