20 Jul BLOG: Changing Futures – New Partnership with Surrey County Council

As part of the ‘Changing Futures’ Government initiative (jointly funded by the Department of Levelling Up, Housing and Communities and The National Lottery), Surrey County Council has partnered with a number of charities and voluntary organisations like us, to deliver an assertive outreach programme for people with multiple disadvantage over the next two years.
They are funding two full-time positions to work from Oakleaf to deliver this key programme, which means we are very pleased to welcome our first ‘Bridge the Gap’ Worker, Christina (pictured below) to the Oakleaf team and are looking to recruit a second!

The Bridge the Gap Alliance is a group of third sector providers that have joined together to provide a specialist, relational model of trauma-informed outreach support for individuals with multiple disadvantages in Surrey and support them with rebuilding their lives.
Our team will be providing support on a one to one basis, 4-8 hours per week per client, from 10am – 8pm Monday to Friday, and 9am – 12pm on weekends; covering a variety of areas, including but not limited to.

- Assistance with finding and/or sustaining accommodation
- Physical and mental health care
- Substance use and harm reduction
- Financial arrangements including benefits applications
- Form filling, health appointments
- Assisting with daily living tasks
- Referrals to other support services.
